2019 Novel Coronavirus (2019-nCoV)
Basics
A novel (new) coronavirus (named "2019-nCoV") that was first detected in Wuhan City, Hubei Province, China
A virus related to bat coronaviruses and the Severe Acute Respiratory Syndrome (SARS) coronavirus
Believed to evolve in animals; coronaviruses, some causing illness in people and others that infect animals, including camels, cats, and bats
Initial spreading thought to be from animal to man via exposure to a large seafood and animal market, but recent epidemiologic surveillance suggests that the virus may have originated elsewhere and was brought into that marketplace.
Most new cases have not had animal market exposure, suggesting person-to-person spread is occurring.
Exposure to people with a travel history to Wuhan, China
Spread via respiratory droplets produced when an infected person coughs or sneezes
Similar as for the influenza virus; hand washing, avoid others if ill.
Obtaining the influenza vaccine; while not protective against the 2019-nCoV, will help the much greater risk of severe illness or death from influenza.

Rumors, misinterpretations, and conspiracy theories are being transmitted at a rate far greater than that of the coronavirus itself. While the situation is evolving rapidly and things can change quickly, our understanding of the illness has also grown in a remarkably short period of time.

Coronavirus (2019-nCov): What You Need to Know
The "2019 novel coronavirus" (2019-nCoV) is a new coronavirus and the responsible agent for an outbreak of pneumonia cases in Wuhan City, Hubei Province, China. Initially reported to the World Health Organization (WHO) on December 31, 2019, it is reportedly linked to a large seafood and animal market.
The situation is rapidly evolving. There have been thousands confirmed human infections and 106 deaths as of January 27, 2020. Cases have been confirmed in at least 16 countries, including the United States and Canada. For an up-to-date heat map of global cases, click here.
What to look for:
The CDC criteria for patients under investigation (PUI) for 2019-nCoV include one of the following 2 scenarios.
Scenario 1:
- - Fever AND symptoms of lower respiratory tract illness (eg, cough, shortness of breath) AND
- - In the last 14 days before symptom onset, a history of travel from Wuhan City, China, OR in the last 14 days before symptom onset, close contact with a PUI for 2019-nCoV while that person was ill.
- - Fever OR symptoms of lower respiratory tract illness (eg, cough, shortness of breath) AND
- - In the last 14 days, close contact (as defined above) with an ill laboratory-confirmed 2019-CoV patient.
Note: Fever may not be present in some patients, such as those who are very young, elderly, immunosuppressed, or taking certain fever-lowering medications. Clinical judgment should be used.
What else could it be:
The differential diagnosis of the 2019 novel coronavirus includes other etiologies of lower respiratory tract infection.
Viral infection:
Bacterial pneumonia, eg:
- - Streptococcus pneumoniae pneumonia
- - Haemophilus influenzae pneumonia
- - Moraxella catarrhalis pneumonia
Atypical bacterial pneumonia, eg:
If you Suspect 2019-nCoV:
See WHO for specific guidance for suspected 2019-nCoV infection. Refer to the US CDC for additional guidance.At present, testing for 2019-nCoV in the United States can only be carried out by the CDC.
No vaccine or specific treatment for 2019-nCoV is available; care is supportive.
